By submitting this form, I agree to the following terms:

By my participation in any SSASO event, I, for myself, my heirs, executors, administrators, and successors HEREBY RELEASE, WAVE AND FOREVER DISCHARGE The Corporation/ SSASO, SSASO club individual board member or directors, or any venue, race organizer, business or land owner(s) that hosts a SSASO event. Further all other officials, volunteers, servants, contractors, representatives, sponsors, successors and assigns OF AND FROM ALL claims, demands, damages, costs, expenses, actions and causes of action, whether in law or equity, in respect of death, injury, loss or damage to my person or property HOWSOEVER CAUSED, arising or to arise by reason of my participation in any club event, whether as spectator, participant, competitor, volunteer or otherwise; whether prior to, during or subsequent to the event AND NOTWITHSTANDING that same may have been contributed to or occasioned by the negligence of any of the aforesaid.

I FURTHER HEREBY UNDERTAKE to HOLD AND SAVE HARMLESS and AGREE TO INDEMNIFY all of the aforesaid from and against any and all liability incurred by any or all of them arising as a result of, or in any way connected with my participation with SSASO or a SSASO event. BY SUBMITTING this FORM, I ACKNOWLEDGE HAVING READ, UNDERSTOOD AND AGREED to the above.

I ACCEPT THE RISKS & any consequences involved in the participation of any SSASO event. I state that my dog(s) do not have a bite history. My dog(s) is/are in excellent medical health and is/are up to date on required vaccinations. I agree to abide by the rules of the event(s) and agree to follow all club/public health related protocols or policy.
